The Premier League and its clubs are committed to a process of consultation with supporters. Part of this process is the National Fan Survey which covers supporters of all 20 Premier League clubs.

We invite you to take part in the survey and we very much hope you will take this opportunity to provide your feedback on your experiences of the game.

The questionnaire should take about 20 minutes of your time. Please complete the survey before 5pm on Friday 21st November to ensure entry into the prize draw.
